Forms
The link that follows is a PDF form that includes instructions. After you have filled it out, be sure to save it as a new file, and then send that file as directed along with any other documents indicated in the form. If you don’t have all of the certificates required for Level 2, fear not. You have plenty of time.
The ICS Forms are maintained by FEMA. As a result, we do not distribute a copy directly as they are not within our control to keep updated versions. We recommend operators download them from the FEMA website at the link above, and keep printed copies ready to go.
The typical forms used by amateur radio operators are: ICS-205, ICS-211, ICS-213, and ICS-214
Note, ICS-309 is commonly used by radio operators, though it is not a formal ICS document as of 2026 (as noted by not being in the range of 200 through 299). This may change due to its popularity. For now we offer a version we find useful: ICS-309.

Digital Modes
Download and register the WinLink program so you are ready to participate in this digital mode. It operates through telnet (Internet) or RF so the transition from one to the other is painless.
This digital mode is easily used with data interface between radio and computer, or using speaker and microphone. The suite consists of at least, fldigi, flamp, flmsg, and flwrap. You may add more functionality from the other related applications as it becomes more familiar.
